How to Refer
Providers:

Complete the online referral form.

Fax or email the client's active medication list, updated progress reports, and any available clinical reports when appropriate and available.
Self Referral:
-
If you are a potential client, we prefer that you be referred by someone involved in your care. However, if you do not have someone who can make a referral, you are welcome to self-refer by calling our admissions line at 443-907-5831
